How can I read flattened form field check boxes?

   I have a PDF form that originally had form fields, filled out and rendered by a customer.  It's been flattened so the form fields no longer exist (FormFieldCount=0).

I need to read the data from it.  I can extract the text easily enough, but the form has several check boxes.  How can I determine if these boxes are checked or not?
