Annotations don't render

I am adding an annotation with AddTextMarkupAnnotation(), the function succeeds (returns 1) and if I save the file to a PDF and open it in Adobe Reader I see my annotation so I know it's working, but then the image returned by RenderPageToString() doesn't include the annotation.  Is this a known limitation, or am I doing something wrong?
