All PDFs come up Insufficient Data For an Image.

We had problems with 1 PDF (for some reason the field count dropped to 0 and threw a runtime error) being generated using version 5.12.  We upgraded to version 5.22, and now every PDF being generated comes up with Insufficient Data for an Image.
 
Has this sort of issue been reported before?

It looks like the new version fixes it for the PDFs that were working fine on version 5.12.
 
I'm testing right now to determine if it fixes the problems that we had for the one erroring PDF on version 5.12.
