Ephraim,

Yes.  Checkboxes in the PDF specification have no concept of attached text so you must draw it manually.

No example code shows Text next to a Check Box/Radio Button

I would have thought that SetFormFieldCaption or SetFormFieldValue would do the trick, to no avail.

Am I supposed to just Draw Text arounfd the FormField?
